Implements health programmes within his/her geographic area or field of responsibility/ expertise.
Referral system management and clinical follow-up
- Coordinates the timely and safe referral of acute and urgent patients according to ICRC/MoHE referral criteria and Standard Operating Procedures (SOPs) from Primary Health Care facilities (Razeh, Munabbih) and Al Talh Hospital to higher-level facilities, ensuring proper authorization and documentation.
- Ensures that referral decisions are made and documented in line with agreed ICRC/MoHE referral criteria and SOPs, and supports clinical staff to apply these consistently and appropriately.
- Ensures that standardized referral forms, authorizations and discharge summaries are used, properly filed and accessible, and facilitates feedback from referral hospitals to referring clinicians.
- Conducts systematic and ongoing follow-up visits to referred patients at receiving hospitals to verify that appropriate and necessary care is provided, identify barriers to care and support communication between patients, families and clinical teams.
- Maintains updated and confidential records of all referred patients according to ICRC data-protection rules and procedures.
Data collection, monitoring and reporting
- Maintains and regularly updates the referral database for all referred patients and produces monthly statistics and narrative analyses for the Hospital Project Manager and health team.
- Produces regular statistics, trend analyses and narrative reports on referral activity and outcomes as required by the Hospital Project Manager, delegation and planning cycle.
- Supports the generation and use of referral data for monthly, quarterly and annual reporting, including contribution to Planning for Results (PfR) and hospital programme reviews.
- Contributes to the Project and Activity Management (PAM) for the activities S/He is responsible for.
Hospital clinical support and quality assurance
- Supports the nursing and clinical teams in ensuring that high-quality care is delivered to admitted patients, following ICRC nursing guidelines and international standards, in close cooperation with the Hospital Project Manager and Health Field Officer.
- Promotes a clean and safe patient environment and supports monitoring that hygiene conditions are regularly measured and maintained in line with ICRC standards.
- Ensures the national and ICRC protocols are followed and adapted to the circumstances.
- Works in close collaboration with the Health Field Officer responsible for the Al Talh hospital programme to ensure that referral practices are consistent with agreed clinical standards and hospital priorities.
